CONC is providing ER through a Case Coordination model along with provision of material aid to ease financial crisis. It has a major focus on assisted referral to address underlying issues impacting on the individuals and/or families financial resources. Some outlets also provide additional group activities such as community meals and community pantries. All outlets have additional wrap around services to enhance outcomes for ER clients.
